01 Gentiva Personal Care Operations · Atlanta, Georgia-based; personal care operations across seven states, including entry into Texas $350M
Addus acquired the personal care operations of Gentiva for an aggregate cash purchase price of approximately $350 million, subject to customary adjustments. The acquired operations added roughly $280 million of annualized revenue across seven states and gave Addus its first personal care presence in Texas, making it one of the largest personal care providers in the country.

Addus acquired Tennessee Quality Care, owned by American Health Partners, for a cash purchase price of $106.0 million. The transaction added approximately $40 million of annualized revenue and gave Addus the ability to provide all three levels of care (personal care, home health and hospice) across Tennessee.

03 JourneyCare · Illinois $72.5M
Addus acquired JourneyCare, Inc. for approximately $72.5 million, funded through a combination of a $35.0 million draw on its revolving credit facility and available cash on hand. The acquisition added hospice services in Illinois, expanding Addus's hospice footprint in the Chicago area.

04 Queen City Hospice · Ohio $192.0M
Addus acquired Queen City Hospice, LLC and Ohio Valley Hospice, LLC (together, Queen City Hospice) for a cash purchase price of $192.0 million, representing $162.8 million of value net of expected tax benefits. The Ohio-based business added approximately $56 million of annualized hospice revenue and was expected to be immediately accretive.

Addus completed the acquisition of County Homemakers, Inc. for a purchase price of approximately $15.8 million, adding personal care operations in Pennsylvania.

06 Hospice Partners of America · Birmingham, Alabama-based; hospice locations across Idaho, Kansas, Missouri, Oregon, Texas and Virginia $130.0M
Addus acquired Hospice Partners of America, LLC, a Birmingham, Alabama-based hospice provider, for a cash purchase price of $130.0 million, representing $118.4 million of value net of expected tax benefits. Hospice Partners served an average daily census of about 1,000 patients across 21 locations in six states, with roughly $55 million of annualized revenue.

07 Ambercare Corporation · New Mexico $40M
Addus HealthCare acquired all of the outstanding stock of Ambercare Corporation, a New Mexico provider, for a cash purchase price of $40 million (before a purchase price credit for estimated cash). Ambercare served approximately 2,600 consumers through 15 locations across New Mexico and generated about $57 million of 2017 revenue, giving Addus personal care, home health and hospice in the state.

08 South Shore Home Health Care and Acaring Home Care · New York City metropolitan area (Northeast) $21.8M
Addus acquired the securities of South Shore Home Health Service and Acaring Home Care for a total purchase price of approximately $21.8 million. The businesses produced about $52 million of 2015 revenue and gave Addus a growth platform in the New York City metropolitan area and its first significant presence in the Northeast.
